Levels vs Wondr Health
Levels is a metabolic health benefit for your whole population, with a GLP-1 companion program for members on medication, backed by an IRB study of 62,236 members. Wondr Health is a 52-week video curriculum with coaches, embedded in Blues plans since its Naturally Slim days, that added GLP-1 access with configurable cost-sharing in 2025.

The short answer
Wondr teaches a class. Levels shows each member their own biology.
Wondr Health sells a year of weekly video lessons, group and 1:1 coaching, and an optional connected scale, billed only for members who engage, often switched on inside a Blues plan. Its 2025 tier, Wondr Advanced, adds obesity clinicians and a menu of GLP-1 cost-sharing designs, from full self-pay to full coverage, with prior authorization and step therapy. There's no glucose, lab or body-composition data in it. The published weight outcomes are self-reported. Levels gives every member CGM, labs, food logging and personalized guidance, adds a companion program for anyone on a GLP-1, and publishes the study behind it, limitations included. One tracks who watched the class. The other tracks what changed in the blood.
Levels fits when
You want results that outlast the prescription
- You want to measure the program in biomarkers, not attendance and self-reported weight.
- You want members to see cause and effect in their own glucose, not a weekly lesson on eating slowly.
- You want to see the evidence. Ours is a 62,236-member IRB study, and every cohort size is on the page.
- You already cover GLP-1s through your PBM and want support around them, not a new set of coverage tiers.
Wondr Health fits when
You want what your carrier already has on the shelf
- You're on BCBS Illinois, BCBS Texas or Independence Blue Cross and want a program you can switch on through the plan, billed as a claim.
- You want a low-cost curriculum for tens of thousands of members, including retirees, and pay only for those who engage.
- You don't cover GLP-1s and want prior authorization, step therapy and self-pay access packaged as "we offer something."
- You're comfortable with self-reported weight as the outcome measure and company-authored evidence.

Wondr Health entries come from wondrhealth.com, its health-plan, employer and consultant pages, Blues-plan sell sheets and its peer-reviewed papers, read September 2026. They describe what Wondr publishes. Where they publish nothing we say "not published" rather than claim the product can't do it. Wondr's ROI and GLP-1 outcome figures are its own reporting. Levels figures come from our IRB-approved study at levels.com/pro/outcomes, which is observational, participant-funded and has no control arm. The cohort size sits next to every number. If a row is out of date, tell us and we'll fix it.
What it costs
Where the money goes, and who carries the risk.
Neither company publishes employer pricing. Wondr bills per engaged member, so the total moves with participation. What you can compare is the shape of the deal: who it covers, what the fee rides on, and what each vendor's own numbers say.
Levels · population benefit
62,236
consented members in the study behind the program. 1.15B glucose readings across 3.5 years.
- One benefit for the whole population, with the GLP-1 companion program included for anyone on medication, at whatever coverage level you set.
- Works with the prescribers and PBM you already have. No second clinic, no medication decisions handed to a vendor.
- Pricing is scoped to your population in consultation. The study is public before you ask.
Wondr Health · pay per engaged member
Not published
for the fee. ROI figures come from one unnamed plan.
- Fees ride on engagement, defined by Wondr. Guarantees are "tied to 5 to 15% weight loss"; how they're measured and what's at risk isn't published.
- 4.6x ROI and $1,154 in net savings per participant, from one unnamed health plan's internal 28-month analysis with no disclosed n. The same source was cited at 1.4x in 2021.
- A GLP-1 cost-sharing menu from full self-pay to full coverage, so the vendor teaching the class also decides who gets the drug and how much they pay for it.
The bill you already pay
Half of it stops early.
About half of members stop GLP-1 therapy within the first year, and most of the lost weight comes back. That's from the Peterson Health Technology Institute's December 2025 report and the STEP 1 trial extension, not from us. Careful prescribing changes who starts. Changing daily behavior changes who finishes, and what's left a year after the last dose.
Being fair
Where Wondr Health is the better call.
Three situations where we wouldn't recommend Levels over Wondr Health. Better you hear it here than mid-RFP.
It's already in your plan.
If you're on BCBS Illinois, BCBS Texas or Independence Blue Cross, Wondr is a line item you can switch on and bill through claims. Levels is a new vendor relationship, and consultations start with your population.
You need something cheap for everyone.
A video curriculum with pooled coaching costs less per head than sensors and blood draws, and Wondr has run it at scale since 2007, including for retirees over 65. If price per eligible is the constraint, Wondr wins it.
You don't cover GLP-1s and want the controls.
Wondr Advanced packages prior authorization, step therapy and self-pay access for plans that exclude the drug. Levels works with the coverage you have. It neither gates nor brokers medication.
If one of those is you, take the Wondr meeting, and ask for the n behind the ROI. Come back when you want to measure the program in blood markers, and a study your consultant can pull apart.
Working alongside
You can run both. Many populations should.
Wondr is a curriculum your plan may already include. Levels is a measurement and behavior layer for everyone. Nothing here asks you to cancel a carrier program. The open question is whether you can tell it's working, and what happens to GLP-1 members once they stop.
- 01
Start with the members Wondr can't measure
Launch the Levels wellness benefit to all eligible members. Anyone in a carrier program stays exactly where they are, with glucose and lab data added.
- 02
Wrap the GLP-1 members you already pay for
Add the companion program alongside your PBM coverage, from first dose through discontinuation. CGM and labs show what a video class can't.
- 03
Read the participation report at renewal
Eligibility, participation and spend in one view. Decide on your own numbers.
Questions we get
Four straight answers.
- Is Levels a replacement for Wondr Health?
- Not one for one. Wondr is a year-long video curriculum with coaching, usually switched on through a carrier. Levels is a metabolic health benefit built on CGM, labs and personalized guidance. If Wondr is free inside your plan, keep it. The question is whether you can tell it's working.
- Does Levels prescribe GLP-1s?
- No. Your PBM and your members' clinicians do. Levels works before medication, with CGM, labs and guidance, and alongside it, with nutrition, movement and side-effect support. The point is what's still true a year after the last dose.
- How does the evidence compare?
- Ours is an IRB-approved longitudinal study of 62,236 consented members and 1.15 billion glucose readings over 3.5 years. Cohort sizes, methods and limitations are at levels.com/pro/outcomes. Wondr's is seven observational papers co-authored by its chief medical officer, on self-reported weight, with no control group. Neither is randomized. Ours measures blood markers. Theirs measures the scale.
- What does Wondr Health cost?
- They don't publish it. You pay per engaged member, billed as a claim or invoiced, with guarantees "tied to 5 to 15% weight loss" whose mechanics aren't public. Consumers pay about $80 a month. Ask how "engaged" is defined, and for the denominator behind the 4.6x ROI.
