Levels vs Omada Health
Levels is a metabolic health benefit for your whole population, with a GLP-1 companion program for members on medication, backed by an IRB study of 62,236 members. Omada Health is a public, multi-condition virtual care company that runs the lifestyle track inside the big three PBMs' GLP-1 programs and markets integrated GLP-1 prescribing through a clinician partner.

The short answer
Omada is built around persistence. Levels is built around what holds after the last dose.
Omada Health wraps a coach, a connected scale and weekly lessons around members who clear a diagnosis or a BMI threshold, and bills your medical plan monthly for each one who stays engaged. Its GLP-1 track sits inside Express Scripts, CVS Caremark and Optum Rx programs, and it now markets integrated GLP-1 prescribing and medication management through a clinician partner. Levels gives every member CGM, labs, food logging and personalized guidance, adds a companion program for anyone on a GLP-1, and publishes the study behind it, cohort sizes included. One scales a coaching program through your PBM. The other shows each member, in their own glucose data, what to change.
Levels fits when
You want results that outlast the prescription
- You want members to see cause and effect in their own glucose within hours of a meal, not on a weekly weigh-in.
- You want something the whole population can use, including members who don't clear a diagnosis or a BMI threshold.
- You want to see the evidence. Ours is a 62,236-member IRB study, and every cohort size is on the page.
- You want a fee scoped to your population that you can forecast, not per-member medical claims that move with engagement.
Omada Health fits when
You want the PBM's default, at scale
- Your PBM is Express Scripts, CVS Caremark or Optum Rx and you want the lifestyle track already built into its GLP-1 prior authorization.
- You want one vendor for prediabetes, type 2 diabetes, hypertension and joint pain, at $0 to the member, billed through medical claims.
- You want a public company's balance sheet behind a multi-year commitment: $222M in cash, no debt, and a profit in its latest quarter.
- You want a cash-pay GLP-1 path with manufacturer pricing, so members can buy a drug you've chosen not to cover.

What each one hands you.
Functionality first, price last. Omada entries come from its own public materials and SEC filings, and we say so where it publishes nothing. Levels entries come from the study behind the program.

Omada Health entries come from omadahealth.com, its GLP-1 medication access page, its trust center, its 2025 annual report and 2026 earnings releases, read September 2026. They describe what Omada publishes. Where it publishes nothing we say "not published" rather than claim the product can't do it. Omada's GLP-1 outcome figures are its own white papers and conference posters. Levels figures come from our IRB-approved study at levels.com/pro/outcomes, which is observational, participant-funded and has no control arm. The cohort size sits next to every number. If a row is out of date, tell us and we'll fix it.
What it costs
Where the money goes, and who carries the risk.
Neither company publishes employer pricing. Omada bills your medical plan per engaged member per month, so the total moves with enrollment. What you can compare is the shape of the deal: who it covers, what the fee rides on, and what each vendor's own numbers say.
Levels · population benefit
62,236
consented members in the study behind the program. 1.15B glucose readings across 3.5 years.
- One benefit for the whole population, with the GLP-1 companion program included for anyone on medication, at whatever coverage level you set.
- Works with the prescribers and PBM you already have. No second clinic, no medication decisions handed to a vendor.
- Pricing is scoped to your population in consultation. The study is public before you ask.
Omada Health · claims-billed
Not published
for the fee. Paid per engaged member, per month.
- Fees ride on engagement, not outcomes. Members who miss four weigh-ins and four app sessions a month drop off the bill, and in PBM programs can lose GLP-1 coverage.
- $4,968 in estimated year-three savings per diabetes and hypertension member, from Omada's employer page. Methodology not published.
- Three GLP-1 access models, including cash pay with manufacturer pricing, so the vendor running your lifestyle track also brokers the drug.
The bill you already pay
Half of it stops early.
About half of members stop GLP-1 therapy within the first year, and most of the lost weight comes back. That's from the Peterson Health Technology Institute's December 2025 report and the STEP 1 trial extension, not from us. Careful prescribing changes who starts. Changing daily behavior changes who finishes, and what's left a year after the last dose.
Being fair
Where Omada Health is the better call.
Three situations where we wouldn't recommend Levels over Omada Health. Better you hear it here than mid-RFP.
Your PBM already picked them.
If your GLP-1 coverage runs through Express Scripts, CVS Caremark or Optum Rx, Omada is the lifestyle track built into the prior authorization. Levels works alongside your PBM, not inside its workflow.
You need one vendor for five conditions.
Prediabetes, diabetes, hypertension, joint pain and now cholesterol, at $0 to the member, billed through claims. Levels is a metabolic health benefit, not a multi-condition clinic.
You want a public balance sheet.
Omada reports quarterly, held $222M in cash with no debt at midyear 2026, and posted a profit in its latest quarter. We're private and don't publish financials.
If one of those is you, take the Omada meeting. Come back when you want the whole population to see their own glucose, and a study your consultant can pull apart.
Working alongside
You can run both. Many populations should.
Omada is scoped to members who clear a diagnosis or a BMI threshold, and lives inside your PBM program. Levels is scoped to everyone. Nothing here asks you to cancel a PBM program. The open question is what the rest of your population gets, and what a member's glucose looks like a year after the last dose.

Start with the members Omada doesn't cover
Launch the Levels wellness benefit to all eligible members. Anyone enrolled in a PBM lifestyle track stays exactly where they are.
- 02
Wrap the GLP-1 members you already pay for
Add the companion program alongside your PBM coverage, from first dose through discontinuation. CGM and labs show what a scale can't.

Questions we get
Four straight answers.
- Is Levels a replacement for Omada Health?
- Not one for one. Omada is a multi-condition coaching program billed through your medical plan, often required by your PBM for GLP-1 coverage. Levels is a population benefit that works with the prescribers and PBM you have. Many employers need the second even if they're committed to the first.
- Does Levels prescribe GLP-1s?
- No. Your PBM and your members' clinicians do. Omada markets integrated GLP-1 prescribing through a clinician partner. Levels works before medication, with CGM, labs and guidance, and alongside it, with nutrition, movement and side-effect support. The point is what's still true a year after the last dose.
- How does the evidence compare?
- Ours is an IRB-approved longitudinal study of 62,236 consented members and 1.15 billion glucose readings over 3.5 years, with cohort sizes, methods and limitations at levels.com/pro/outcomes. Omada's diabetes prevention program has a 599-person randomized trial and CDC recognition. Its GLP-1 figures are company white papers, and the 12-month regain result rests on 95 members. Read both.
- What does Omada Health cost?
- They don't publish it. Omada bills your medical plan monthly for each member who meets engagement criteria, using its own CPT code, and management describes the GLP-1 product as roughly 2.5 times the price of its prevention program. Ask for the engagement definition and the per-member rate in writing.
